A 28 year old breastfeeding mom G3 P3, 3 months postpartum, wants to continue breastfeeding. She wants OCP immediately because she does not want to become pregnant too soon.
What type of OCP is appropriate for this mom?

A progestin- only OCP like Heather 0.35mg 1 tab po q d can be started after ruling out pregnancy and previous drug allergies. The pill has to be taken same time daily without breaks. Educate her that if she’s late in taking the pill 3 hours or more, she must use backup contraception for the next 7 days or abstain from sexual intercourse for the next 2 days while continuing to take the pill as normal. Clinical Pearls:
-Progestin only OCP does not suppress breast milk production.
-It thickens cervical mucus which prevents sperm from passing through.
-Almost 100% effective in preventing pregnancy in combination with exclusive breastfeeding and if taken correctly.
-It thins the lining of the uterus preventing implantation.
-It does not prevent ovulation. (AAFP 2016a, AAP 2015)
